Cyanosis
Cyanosis : is a bluish discoloration of the skin and mucous membranes ,Cyanosis can be seen when the deoxygenated hemoglobin in the blood reaches between 4-6 gm/dl ,normally 80-87% oxygen saturation could give rise to clinically apparent cyanosis .in cases of anaemia cyanosis may be seen when oxygen saturation may be as low as 60%.
Ask about the onst of cyanosis did it happen suddenly or has it been appearing over a long period ? a sudden onset may be due to pulmonary emboli or severe asthma , Ask about any associated symptoms as dyspnea chest pain ,and cough , Ask about any past medical history of respiratory or cardiac disease .Ask about any medication that may cause abnormal hemoglobin ,Don,t forget occupational and smoking history.
Examination : Check the pulse and blood pressure if week and blood pressure is low ,there may be a circulatory shock .fever in case of pulmonary infection ,look for clubbing of fingers and growth retardation in children The combination of clubbing and cyanosis is frequent in congenital heart disease and may also occur in pulmonary disease ( lung abscess, bronchiectasis ,cystic fibrosis and pulmonary arteriovenous shunts ).Look for the tongue for central cyanosis Look for the neck for raised JVP in heart failure and working accessory muscles in severe asthma , Examine the chest : Note for barrel chest ,kyphosis,scoliosis,scars . Poor chest expansion in COPD and asthma ,unilateral reduced chest expansion with lobar pneumonia , dullness to percussion over an area of consolidation ,localised crepitations may be heared in lobar pneumonia wide spread crepitations in bronchopneumonia and pulmonary edema .bornchial breathing may be auscultated over an area of consolidation . (Q) What are the types of cyanosis ?
(A) (1) Central cyanosis : (a) cardiac causes : cyanotic or congenital heart diseases e,g fallot tetralogy ,transposition of great artries , and trancus arteriosus , (b) Respiratory causes : pulmonary edema , pulmonary embolism , high altitude sickness ,severe pneumonia ,severe asthma , COPD, ARDS (C) Causes in the blood : methemoglobinemia ,sulfhemoglobinemia and polycythemia .
in central cyanosis limbs appear warm to touch and cyanosis does not disappear on warming of the hands .
(2) Peripheral cyanosis : occurs with sluggish or slowing of circulation , may be seen in heart failure and shock ,exposure to cold temperature ,arterial vascular occulsion of one limb ,limbs appear bluish and cold to touch ,warming may resolve the cyanosis .
(Q) What is the differential cyanosis ?
(A) Cyanosis and clubbing of the lower extremities with normal upper extremity nail beds it is diagnostic of patent ductus arteriosus with pulmonary hypertension.

Signs of loss of lung volume : as seen in this x-ray :
- Generalized shift of the mediastinum
-The trachea and heart gets shifted towards the atelectatic lung .
Elevation of the diaphragm : the diaphragm moves up and the normal relationship between left and right side gets altered .
-Drooping of the shoulder .
-Crowding of ribs : the interspace between ribs is narrower compared to the opposite side

Daily Syndrome : Red man syndrome
Vancomycin causes several different types of hypersensitivity reactions, ranging from localized skin reactions to generalized cardiovascular collapse. The most common adverse reaction, the "red man syndrome" (RMS), is a rate-dependent infusion reaction, not a true allergic reaction
Signs and symptoms — RMS is characterized by flushing, erythema, and pruritus, usually affecting the upper body, neck, and face more than the lower body. Pains and muscle spasms in the back and chest, dyspnea, and hypotension may also occur. Otherwise unexplained hypotension has been reported
If red man syndrome appears then the vancomycin infusion should be discontinued immediately. A dose of 50 mg diphenhydramine hydrochloride intravenously or orally can abort most of the reactions. Once the rash and itching dissipate, the infusion can be resumed at a slower rate and/or at a lesser dosage. Hypotension will require intravenous fluids and, if severe, vasopressors may be needed, each intravenous dose of vancomycin should be administered over at least a 60 min interval to minimize the infusion-related adverse effects

Recurrent mouth ulcers or Aphthous stomatitis : The disease causes painful ,round ulcers to develop on the linings of the cheecks ,lips ,the tongue or the base of the gums ,the tendency to develop these ulcers is inherited ,ulcers can be associated with other diseases particularly connective tissue diseases such as lupus or Behcet,s syndrome which cause symptoms on the eyes and genitals as well as the mouth the ulcers can be single or multiple and they are recurrent ,multiple ulcers are scattered along the lining of the mouth ,not clustered , the cause of recurrent ulcers is not known but most theories involve immune abnormality ,certain blood diseases ,vitamin and mineral deficiencies ,allergies ,trauma and crohn,s disease cause similar ulcers ..
Recurrent aphthous stomatitis are classified into 3 categories :
(1)Minor ulcers : less than 1 cm and do not leave scar , usually heal within 2 weeks (2) Major ulcers : 1/2 inch or more in diameter take longer time than minor ulcers to heal and may leave scar . (3) Herptiform ulcers ;
are clusters of dozen of small ulcers (this form is rare ).
people tend to have 2-6 ulcers per episode and have several episodes each year . If the ulcers become more frequent or severe ,or accompanied by other symptoms such as rashes ,joint pain ,fever or diarrhea or are larger than about 1/2 inch in diameter ( blood diseases ,drug reactions ,connective tissue disorders and skin disorders should be considered..
Treatment : Correction of anaemia ,iron ,folate or B12 deficiency the ulcers show improvement when these deficiencies are treated ..
Treatment focuses on reliving symptoms ,rinsing with warm water solution and eating bland foods can minimize discomfort ,anesthetic medications or over the counter agents that are placed directly on the sores to coat them also may help . People with more severe disease may need steroid medications placed on the lesion these may shorten the healing time of the ulcers and preventing them from becoming larger

Weil syndrome, the severe form of leptospirosis, primarily manifests as profound jaundice, renal dysfunction, hepatic necrosis, pulmonary dysfunction, and hemorrhagic diathesis. Pulmonary manifestations include cough, dyspnea, chest pain, bloodstained sputum, hemoptysis, and respiratory failure
A good clinical history is often the key to accurate diagnosis in leptospirosis. Important features include exposure history and a clinical picture consistent with the disease.

The exposure history may reveal direct contact with body fluids or organs of infected animals, or indirectly (eg, via contaminated soil or water). Direct exposure often occurs in occupational cases, while indirect exposure is more typical of cases contracted during travel or recreational activities rivers .
Abdominal pain with diarrhea or constipation (30%), hepatosplenomegaly, nausea, vomiting, and anorexia are also seen. Acalculous cholecystitis may be seen rarely but is clinically significant. Subconjunctival hemorrhage is the most common ocular complication of leptospirosis, occurring in as many as 92% of patients.

Renal manifestations include hematuria. Oliguric or anuric acute tubular necrosis may occur during the second week due to hypovolemia and decreased renal perfusion.The onset of mental status alterations indicates progression to parenchymal involvement of the cerebral cortex with meningo-encephalitis, heralding a high mortality risk. For severe leptospirosis, intravenous penicillin G has long been the drug of choice, although the third-generation cephalosporins cefotaxime and ceftriaxone have become widely used.Supportive therapy and careful management of renal, hepatic, hematologic, and central nervous system complications are important. Management

Patients should be managed in a monitored setting because their condition can rapidly progress to cardiovascular collapse and shock. Access to mechanical ventilation and airway protection should be available in the event of respiratory compromise. Continuous cardiac monitoring should be attained; arrhythmias, including ventricular tachycardia and premature ventricular contractions, as well as atrial fibrillation, flutter, and tachycardia, can occur.Renal function should be evaluated carefully and dialysis considered in cases of renal failure. In most cases, the renal damage is reversible if the patient survives the acute illness. Early initiation of hemodialysis or peritoneal dialysis considerably reduces mortality risk.Corticosteroid therapy is controversial.Treatment with high-dose pulsed methylprednisolone (30 mg/kg/d, not to exceed 1500 mg) has been used successfully to treat patients with leptospiral renal failure without dialysis. This approach may have be beneficial in resource-poor areas where dialysis is unavailable and would involve lengthy medical transport. The use of renal-dose dopamine in conjunction with steroids or diuretics has also been described.

Pulse-dose steroids may also play a role early in the management of severe pulmonary disease. Patients with Weil syndrome may need transfusions of whole blood, platelets, or both. Ophthalmic drops of mydriatics and corticosteroids have been used for relief of ocular symptoms.

Pruritus(Itching)

Pruritus is a disagreable sensation that provokes a desire to scratch .Pruritus is an important skin symptom and occurs in dermatological diseases such as atopic eczema ,many cases can be attributed to dry skin , many other causes ,scabies ,dermatitis herpetiformis ,pruritus vulvae or ani ,insect bites ,pediculosis ,contact dermatitis ,drug reactions
,urticarial eruptions of pregnancy ,psoraisis,lichen planus ,lichen simplex chronicus ,exfoliative dermatitis .
Persistant pruritus not explained by cuteanous disease should prompt a staged workup for systemic causes ,the most common cause of pruritus associated with systemic disease is uremia in conjunction with hemodialysis In primary biliary cirrhosis the disease starts insidously most frequently without jaundice ,jaundice may never develop but in the majority appears within 6 months to 2 years of the onset of pruritus ,. Causes of generalized pruritus :
-Obstructive liver disease -Hematological : Iron defeciency anemia , polycythemia -Endocrine : Hyperthyroidism, Hypothyroidism ,Diabetes mellitus . Chronic renal failure - Malignancy : Internal malignancies , lymphoma Drugs: Morphine . Other :
Pregnancy -senelity .Both pruritus of uremia and the pruritus of liver disease may be helped by phototherapy with ultraviolet B or PUVA.Naltrexene and nalmefene have been shown to releive the pruritus of liver disease . Naltexone is not effective in pruritus associated with renal failure but gabapentin may be effective

Daily Syndrome : Young Syndrome Young syndrome is a condition characterized by male infertility, damaged airways in the lungs (bronchiectasis), and inflammation of the sinuses (sinusitis).
Male infertility in Young syndrome is secondary to obstructive azoospermia, a condition in which sperm are produced but do not mix with the rest of the ejaculatory fluid, due to a physical obstruction in the epididymis (tube through which sperm exit the testis). This results in nonexistent levels of sperm in semen.
Young syndrome is typically diagnosed in middle-aged men who undergo evaluation for infertility. As the signs and symptoms of Young syndrome are similar to cystic fibrosis (CF), part of the diagnosis process may include ruling out CF.
Although the exact cause of Young syndrome has not been identified, it is believed to either be related to childhood exposure to mercury or genetic factors. While there is no one treatment for Young syndrome, management involves treatment of sinus and lung infections. Fertility treatment may also be an option, including surgery to remove the obstruction in the epididymis (vasoepididymostomy)

The diagnosis of Young syndrome is based on the presence of signs and symptoms and the absence of other similar conditions, such as cystic fibrosis and primary ciliary dyskinesia

Case story (From the work experience) : A 6 years old boy brought to the ED in Emaba fever hospital refered from the university hospital as meningococcal meningitis , the child was febrile , drowsy , has dessminated dark violaceous skin rash associated with swollen eye lids , inflammed red both eyes , swollen lips and inflammed buccal mucosa with cracking and erosions . The CSF analysis revealed : cells 10/cmm , sugar : 76mg/dl , protein 18 mg/dl , no organism detected on gram stain No history of convulsion , Past history was unremarkable except for a distant history of tonsillectomy and adenoidectomy , the family history was not contributory (Q) Do you agree for such a diagnosis? The story started 5 days earlier by fever , sore throat dry cough and vomiting once , received empirical ceftriaxone from a private dispensary together with paracetamol as an antipyretic without adequate satisfactory response . On the fourth day a maculopapular rash appeared on the face , and trunk , then distributed to the abdomen palms ,soles and lower limbs the rash was burning but non itchy , the eyes ,lips and genitlia were involved . He was provisionally diagnosed as measles and isolated in our hospital ward for one day then discharged on the parents request to continue treatment and isolation at home . One day later the general condition deteriorated more , fever persisted and the rash became more disseminated and more dark , transferred to the university pediatric hospital where LP IS DONE . Physical examination showed a drowsy child , temperature : 38.5 C , O2 saturation : 97% on room air , pupils equal ,about 5mm , reacted briskly to light , pulse :120/m no lymphadenopathy , RR: 24/m , no jaundice , mouth examination: swollen lips with inflammed ulcerated buccal mucosa , with oral moniliasis Skin examination : revealed flat irregular purpuric macules with some blistering lesions some lesions have a cyanotic center surrounded by pink erythema neck lax with no signs of meningeal irritation , GCS: moderate 10/15 E1V4M5 , : Chest: harsh vesciular breathing , Heart : normal S1, S2 . Abdomen : soft , Joint examination : normal , Neurological examination : intact motor and sensory power and no cranial nerve affection . Investigations : CBC was normal apart from leukocytosis ( TLC , 13,500) , CRP was positive , ESR :40 Na :129meq/l , k: 3.4 urine examination normal, S.liver and renal profile was normal , Chest x-ray : normal , CT Brain examination : was normal .
What is the differential diagnosis?
(1)Steven jhonson syndrome (2) kawasaki disease (3) Measles (4)Meningococcal meningitis
Discussion of the differential diagnosis : - The fever ,rash and altered mental status raises the possibility of meningococcal meningitis but the rash is characteristically petechial and purpuric reflectig accompaning systemic vasculitis the rash is sometimes similar to that of measles but cough and conjunctivitis are usually abscent , gram stain and culture failed to reveal evidence of meningococcal meningitis , however the short course of antibiotics prior to LP reduces the chance of isolation of bacteria in CSF culture making the diagnosis difficult ,and this is the importance for molecular methods (PCR) for rapid diagnosis of bacterial meningitis . The prodrome of upper respiratory symptoms , fever and painful skin together with the cuteanous and the mucosal lesions which invoved the lips , mouth , and conjunctiva , the iris or target lesion is a pathognomonic diagnosis for erythema multiform which was present early in this patient , the morbiliform rash observed initially was confusing with the measles viral exanthem , however there was no coryza or watery eyes which is present in measles prodroma , and the rash is sometimes itchy but not burning .Kawasaki disease is an acute febrile illness affecting infants and children characterized by bilateral conjunctival congestion , with dryness ,redness and fissuring of the lips also exanthematous rash localized to the trunk with redness of the palms and soles , with desquamation ,also increase CRP and leukocytosis it is excluded by absence of painful cervical lymphadenopathy , there was no arthralgia or arthritis absence of gastrointestinal or uretheral symptoms . and the liver enzymes were normal , The patient was managed as Steven Jhonson Syndrome , the offending drug stopped (ceftriaxone ) , the patient is isolated , I.V. fluids started , dermatologic , and ophthalmolgic consultation , antihistamines are given , skin and mouth care , topical soothing application , short course of coticosteroids ,antibiotics drops and ointments , systemic antibiotics to guard against secondary infection , follow up of the patient was excellent , the patient markedly improved , fever subsided and the rash disquamated over about one week .
Diagnosis : Steven Jhonson syndrome
Conclusion :Patients with Steven jahonson Syndrome may present early in the development of the disease may not be critically ill , the clinician may misdiagnose the condition and discharge the patient , with extraordinary generalized eruption with continuous fever ,inflammed buccal mucosa , and severe purulent conjunctivitis , the case is misdiagnosed by the physician as hemorrhagic measles

Necrotizing fasciitis
This is an acute necrotizing infection of the skin ,subcuteanous tissues and superficial fascia ,it may involve muscle . Clinically there is soft erythema or dusky appearance with severe pain . Scarlet Fever
It is an infectious disease caused by streptococcus pyogenes ( group A strept.) , usually spread by inhalation ,it is most common in children 5-15 years of age . Scarlet fever features a bright red rash that covers most of the body this rash is caused by an erythrogenic toxin released from the bacterium (streptococcus pyogenes) ,it is almost accompanied by sore throat and high fever . The signs and symptoms include :
Red rash : The rash looks like a sunburn and feels like a sand paper ,it appears 12-72 hours after the fever start ,on the face it shows as red cheeks with characteristic pale area around the mouth (circumoral pallor) , it typically begins on the face or neck and spreads to the trunk ,arms and legs ,if pressure is applied to the rash it turns pale ,begins to fade 3-4 days after onset and desquamation begins .the rash may be itchy .
Red lines : The folds of skin around the groin ,axillae ,elbows ,knees and neck usually became a deeper red than the surrounding rash .
Flushed face : the face may appear flushed with a pale ring around the mouth . Strawberry tongue : the tongue look red and is often covered with a white coating early in the disease . Incubation period : 2-4 days .
Rarely scarlet fever can lead to rheumatic fever .
Differnitial diagnosis : kawasaki disease is an important differential especially in its incomplete form ,scarlet fever appears similar to kawasaki disease in some aspects but lacks the eye signs or the swollen red fingers and toes .
Treatment
Penicillin or amoxicillin is the antibiotic of choice to treat scarlet fever. There has never been a report of a clinical isolate of group A strep that is resistant to penicillin. For patients with a penicillin allergy, recommended regimens include narrow-spectrum cephalosporins (e.g., cephalexin, cefadroxil), clindamycin, azithromycin, and clarithromycin.
